Alright, if you want to get absolutely busted in The Binding of Isaac: Repentance, you need a mix of skill, strategy, and sometimes just dumb luck. The key is snagging the right items early and snowballing from there. Prioritize high-damage items like Sacred Heart, Polyphemus, or Brimstone—stuff that completely changes the way you play. Tear multipliers and homing effects stack insanely well, so combine them when you can. If you see an item that gives you extra tears, damage, or homing, grab it without hesitation.
Devil and Angel deals are your best friends, but they both come with different playstyles. If you want raw power fast, take Devil deals and trade away red hearts for high-damage upgrades. But if you can stay healthy and avoid red heart damage, Angel rooms can reward you with some of the strongest defensive and offensive items in the game—like Sacred Heart or Godhead. Either way, you need to make a choice early on and commit to it. Also, don’t sleep on the shops and secret rooms; they can hold game-changing items like Steam Sale, Diplopia, or R Key, which can let you break the run wide open.
Synergies are everything. You want items that play off each other in a way that turns you into an untouchable powerhouse. Think about how Brimstone and Tammy’s Head can clear entire rooms instantly or how something like Mutant Spider plus Monstro’s Lung can fill the screen with projectiles. If you see something that duplicates pickups (like Blank Card + Two of Diamonds or D20 in the right room), you might have just unlocked infinite resources.
There are ways to farm infinite items, like using the D6 to reroll pedestals or exploiting Restock and Deep Pockets to never run out of money. If you ever find yourself with a Perfection trinket, grab a luck-based tear effect like Holy Light or Tough Love and watch enemies melt. Play smart, play aggressive, and once you start spiraling into OP territory, never let go of that momentum.
